Support Services

MUSC Children’s Health care teams include social workers and nurse case managers who assist patients and their families in accessing resources at MUSC Children’s Health as well as well as in their community.

Social Work Support Services

Social workers at MUSC Children's Health are part of a team of dedicated, caring professionals working with you and your child during their visit to the hospital and clinics. They provide services that support your social and emotional needs. Each social worker has a Master’s Degree in Social Work and is licensed by the state of South Carolina to practice Social Work.

Our social workers are available on the MUSC Medical Center campus Monday to Friday from 8:30 a.m. to 5 p.m. and are available 24 hours a day, 7 days a week – on an emergency basis – to assist you and your family during your child's visit to MUSC Children's Hospital. We provide a vast array of services including support, assistance, and referrals for services/resources in the hospital and community. We want to ensure that your needs and the needs of your child are addressed.

Some of the services we provide include:

  • Emotional support to patients and caregivers.
  • Crisis situation intervention.
  • Coping and adjusting to new illness and treatment.
  • Brief grief and bereavement support.
  • Alcohol and drug use/abuse assessment and referrals.
  • Legal issues, such as adoption and custody interventions.
  • Financial needs assessment and referrals.
  • Educational and vocational needs.
  • Discharge planning.
  • Advocacy for patients and families.
  • Coordinating patient care conferences.
  • Assessment/referrals of allegations of child abuse and neglect, domestic violence, physical assaults, and rape or other sexual assaults.
  • Information and referrals for community resources and services.

Social workers are available to all patients and families and can be contacted by hospital staff, patients, family members and community agencies. Our social workers are available on every unit at MUSC Children's Health and in all outpatient clinics. Nurse Case Managers

Nurse Case Managers at MUSC Health are registered nurses (RNs) with at least a 4-year nursing degree as well as years of nursing experience who develop, implement and evaluate individualized patient care plans. We advocate for patient welfare and serve as a liaison between patients, their families and healthcare providers.

Nurse Case Managers coordinate all aspects of the care of individual patients. We ensure proper utilization of services and resources as well. Case managers provide assistance within, between and outside of facilities. Nurse Case Managers are experts at obtaining resources.

We work closely with physicians, nurses, social workers and a wide range of medical and non-medical professionals. Case managers work to meet complex patient needs and facilitate admission and discharge processes.

Case Managers at MUSC Health work holistically, ensuring that care and discharge plans meet the physical, social, and emotional needs of patients.

The role of Nurse Case Managers at MUSC Children’s Health:

  • Identify and address client risk factors and/or barriers to care.
  • Identify client needs, current services and available resources, then connect the client to services and resources to meet established goals.
  • Screen clients and/or population for healthcare needs.
  • Develop a client-focused case management plan.
  • Educate the client/family/caregiver about the case management process and evaluate their understanding of the process.
  • Coordinate patient care conferences.
  • Identify clients’ insurance coverage or other sources of payment for services.
  • Communicate the care preferences of clients, serve as their advocate and verify that interventions meet the client’s needs and goals of treatment.
  • Work with new admissions, assess their conditions and needs in order to develop personalized care plans.
  • Evaluate plans of care and update them on an as-needed basis.
  • Educate patients and their families on how to follow their care plans.
  • Assess discharge needs.
  • Ensure that each patient has insurance coverage for every day in the hospital.
  • Assist patients/families with coping and adjusting to new illness and treatment plans.

Nurse Case Managers at MUSC Children’s Health are available to all patients and families and can be contacted by hospital staff, patients, family members and community agencies. Nurse Case Managers are available on every unit at MUSC Children's Health. Our department is fully-staffed weekdays between 8:30 a.m. to 5 p.m. in the hospital. If you need to reach your Case Manager after-hours or on the weekend, you can ask your child's nurse or doctor to call or page them.
